Task:

You will be caring for Florence/Fred as a GP nurse in the community. Provide a handover as the GP nurse to the QAS officers who will be transporting your patient to hospital. 

Case Study

Florence/Fred has presented to their local GPs practice with increased nausea and vomiting and aches and pain to their back and legs. They have a background of newly diagnosed bowel ca and underwent a bowel resection at the local hospital 8/52 ago. They have been receiving chemotherapy for the past 2/52 which they had been tolerating well until 3/7 ago when they started experiencing increased nausea and vomiting and decreased oral intake. They have a medical history of COPD, Osteoarthritis and Type 1 DM.

The observations on your patient are: Temp 37.4, pulse 96 bpm, BP:110/60, RR26, Sats 91% on 2L 02 via N/Ps

Pain- 7/10 pre analgesia dose 

Abdo assessment- hypoactive bowel sounds 

Vomit of 350mls during the 4 hours the patient has been in the GP clinic, intake of 30mls.

BGL- 5.3mmol/L

Respiratory assessment- Normal lung sounds, equal air entry.
