Description

Gilbert and Brown (2016) states, clinical skills are more than successfully performing tasks. They incorporate a person's assessment, identification of deficits and/or problems, and the ability to critically think to provide solutions. Thus, this assessment is to further your knowledge on clinical decision making and care coordination (interventions) processes, for the provision of care on a person living with illness. 

Aims of this assessment:

to identify one (1) health need (problem) as the Assessment of need, that relates to your selected case study

formulate a plan of care to the identified goal 

implement the provision of care addressing the health need, using supporting resources for each of the environments MICRO, MESO and MACRO of the Socioecological framework (model)

Criteria

Your submitted product through safe assignment will be assessed on the following Criteria:

Accurate nursing assessment using the Corbin and Strauss Illness Trajectory Model reasoned to the goal of management.

Demonstrates critical thinking in the decision making, care planning, care delivery and management which align to the socioecological framework.

Provide rationales for factor(s) that may facilitate (contribute to) or hinder goals in the management of the case.

Communicates effectively in the academic context with appropriate presentation of the case using correct academic language.

Apply the Harvard referencing format and uses current peer reviewed sources to support this assessment

Case Study Instructions

Using the knowledge of the Corbin and Strauss Illness Trajectory Model, your next step is applying this model of care as an accurate assessment to a case study, supporting the trajectory phase and goal of management identified from one of the three selected case videos. You will need to select only one (1) case. Focus your response on one (1) health need (problem) ensuring you are specific to the health event. You are encouraged to consider a goal that is attainable. It is recommended that you prepare for this by reading about: - the Process of Nursing – Assessments -Care Plan – Implementation. This information is found in any Fundamental of Nursing textbook.
