1.Given that Ali and his family have English as a second language, how will you identify if an interpreter is needed?

2.Ali’s mother is going to be completing home practice with Ali. How can you check if she understands the therapy program and what she needs to follow up on?

3.Explain how you would work in consultation with the speech pathologist to prepare resources and activities for sessions with Ali and his mother. Ensure you cover:

a.How will you consider information on Ali’s abilities and interests in choosing which resources are used?

b.How might you adapt existing resources?

c.How will you check to see if the resources are appropriate?

4.Part of the program involves Ali and his mother completing practice at home. How will you explain the importance of home practice and encourage it to be completed? Consider how you will communicate this to the mother given that English is her second language.

5.Six weeks into the weekly program, Ali and his mother begin missing appointments. This occurs three times in a row.  How do you manage this issue?

6.Provide an example of how you would document Ali and his mother’s failure to attend according to workplace policy and procedures. Include the current situation of FTA (fail to attend), your plan of action regarding the FTAs and the outcome for the client if they continue to non-attend.

PART B: Client – Brian

Brian is a 59-year-old man who is experiencing swallowing difficulties following a stroke. You are to assist the speech pathologist with the assessment appointment and arrange the setup of equipment that is needed.

1)You will also be present to assist both the speech pathologist and Brian within the assessment.  

a.How will you support both the speech pathologist and Brian at the assessment appointment?

b.The speech pathologist has asked that you prepare texture modified food and thickened drinks for the assessment. How will you ensure you carry out this preparation safely? Consider the workplace policies or resources to which you may need to follow or refer.

Following the assessment, the speech pathologist has recommended Brian start using some swallowing strategies to assist and improve his dysphagia. You will be working with Brian for 6 weeks in one of the mealtime groups (morning or afternoon) to support him to implement strategies to assist with his dysphagia.

2)Brian has multiple allied health therapies supporting him with his rehabilitation and subsequent appointments.

a.How will you determine Brian’s availability for appointments?

b.Besides supporting Brian with modified food and drinks, what else is important to consider for a person with dysphagia during mealtimes?

c.After a couple of sessions with Brian you notice that he is coughing during and following his drinks. Why would this concern you and what steps will you take to manage this concern?

d.How and when will you share your concerns with the speech pathologist?

e.You notice that Brian is eating his food rapidly and does not seem to be clearing his mouth before taking the next mouthful. All of a sudden, Brian starts to choke – no noise is coming out of his mouth, he is unable to breathe and his face starts to turn blue. How would you respond to this event?

f.How would you document this event according to policies and procedures? Use the following headings:
    
        i) On Examination (O/E):
            Include place and time of observation, situation and how Brian was presenting initially in session.
        ii) Swallowing observation:
            Include what and how eating. What were main concerns?
        iii) Intervention:
            Include what was done in the situation and by whom.
        iv) Plan:
            Include what you recommend should happen next

2)Participating within supervision is important to ensure you are supported in your role and thus can adequately support clients with their complex needs.

a.What does participating in supervision involve?
