Nurses assess and manage risk as an element of their daily practice in order to keep patients safe, and to ensure optimal outcomes. This assessment task is designed to help you apply the concept of risk management to a real-life nursing situation you have observed or experienced.
Identify a situation that you were involved in or observed during clinical placement that required the nursing management of clinical risk. This may be ANY situation where the safety of the patient was potentially at risk.
Common areas of clinical risk include but are not limited to:
• Medication administration
• Fluid management
• Hospital acquired infection
• Blood product administration
• Pressure injury
• Falls
• Delayed recognition of clinical deterioration
Apply the clinical risk management process to your chosen clinical exemplar
Part 1: Establish the Context
Write a description of your clinical exemplar. Describe the clinical setting, the staffing and conditions on the unit, the patient condition and background, and the reason for their admission. The focus of this section should be on any aspects of the context that may have contributed to patient risk, using a systems approach.
This section should be written in first person and does NOT require referencing.
Part 2: Apply the Clinical Risk Management Process
Identification: How can this risk be identified in practice or how is this risk monitored and recorded?
Analysis: How common is this type of risk in practice? What is the significance for patient outcomes?
Evaluation: How can this risk potentially reduced or eliminated in practice? What processes, policies or structures can be put in place to reduce this risk?
Management: What is the role of the nurse in managing or monitoring for this type of risk?
