Task 2.
 
On February 21, 2017 a Beechcraft B200 aircraft crashed into a DFO resulting in 5 people being killed.

Now, imagine that the accident happened today in your state (South Australia).

You have been appointed as the lead investigator for a four person investigation team (including yourself) for this accident and you are preparing for the investigation.  You need to obtain information to help you with your planning, do a risk assessment.  You need to think about the necessary logistics to get yourself and your team from your office to the area and on to the accident site.  You also need to make sure your team has the equipment at the scene to get started on the investigation.

Information

1.To do your planning, you need information about the accident. What information about this accident scene do you have and what additional information would you request?

Risk Assessment – As the lead investigator, you have a responsibility to keep your investigation team safe.

1.What are the hazards that your investigation team will face at this particular accident scene? Describe each hazard and how it will affect or potentially harm your investigation team members.  You should identify at least 10.

2.Using the risk matrix provided, for each hazard, what is the consequence of it, the likelihood of it affecting your team members and the final risk score?

3.What are you going to do to keep your investigation team safe?  Describe 10 controls you will put in place for the hazards identified at this accident scene.

Team

1.For this investigation, what normal roles will you need to fill in your four member team?  Also identify two additional team members as subject matter experts.  For each subject matter expert, write one or two sentences to explain why each expert is needed.  (Assume that the emergency response teams have already completed their tasks).
Logistics

1.You need to get your investigation team to the accident site and make sure that they are able to stay in the area for up to a week.  Assuming you and your team are based in Brisbane, how will you get your team and equipment to the accident site and where will they stay?  How will you ensure your team members are provided with the necessities of life during this time?  How will communications be managed?

Equipment - You need to make sure your investigation team has all the equipment they need to do the investigation.

1.Provide an itemised list of everything in your crash kit.
2.What other equipment will you need at the accident scene? Remember your investigation could be going 24 hours per day for several days.Arrival at the scene
1.Once you get to the scene, how will you explain all of these arrangements to your team?
